Breakfast is not the most important meal of the day

The trials are clear and have been for a decade. What makes this story worth telling is not that the advice is wrong — it is that researchers documented, in detail, how the scientific literature talked itself into believing it.

Start with the part that is genuinely well established: people who skip breakfast are, on average, heavier and in worse cardiovascular health than people who don’t. That association is not in dispute. In one analysis it had been demonstrated so many times over that a cumulative meta-analysis put the combined p-value below 10−42.

Now the part that does not follow: that eating breakfast will therefore make you healthier.

What happens when you run the experiment

A 2019 systematic review in the BMJ pooled every randomised trial it could find. Across seven trials and 486 participants, adding breakfast produced a weight difference of 0.44 kg (95% CI 0.07 to 0.82) in favour of the people who skipped it. Across ten trials and 930 participants, breakfast eaters consumed about 260 more calories a day.

The authors’ conclusion: adding breakfast “might not be a good strategy for weight loss,” and “could have the opposite effect.”

The single best individual trial is Dhurandhar and colleagues: 309 adults with overweight or obesity randomised for 16 weeks to a pamphlet, to eating breakfast before 10am, or to eating nothing before 11am. Compliance was above 92 per cent in both intervention arms — people did what they were told. Treatment assignment had no significant effect on weight (P = 0.77), and there was no interaction with whether people ate breakfast at baseline.

That last detail matters, because the usual escape hatch is “it only works if you’re already a breakfast eater.” The trial tested that directly and found nothing.

The part that is actually new

In 2013, three researchers — Andrew Brown, Michelle Bohan Brown and David Allison — did something unusual. Rather than run another study, they audited how the existing literature cited itself.

They identified two practices. The first they called research lacking probative value: researchers continuing to run observational studies to re-establish an association already proven past any doubt, which they described as a suboptimal use of collective scientific resources. The second was biased research reporting.

The numbers are worth reading slowly. Of 91 articles citing one particular randomised trial, 46 cited it in connection with breakfast and obesity — and only 17 per cent described its results accurately. Of the 29 articles that cited it misleadingly, 28 skewed in favour of breakfast and one against. For a second study, 48 per cent of citations ascribed a stronger inferential meaning than the article warranted. Across observational abstracts generally, between a quarter and half were given more inferential strength than their designs supported.

Their summary: “The belief in the [proposed effect of breakfast on obesity] exceeds the strength of scientific evidence.”

That is not a media failure. That is the scientific literature doing it to itself, in one consistent direction, and somebody counting.

Where breakfast genuinely matters

It would be a mistake to read this as “breakfast does nothing.” Two things hold up well.

Undernourished children. A review of 36 studies found breakfast has a positive effect on children’s memory and attention — and states plainly that the effects “are more demonstrable in children who are considered undernourished.” The foundational experiment, a crossover study of Jamaican schoolchildren, brought each child in twice after an overnight fast, once with breakfast and once without. Stunted and previously malnourished children performed worse on fluency and coding without it. The adequately nourished control children scored higher on arithmetic when breakfast was omitted.

Physiology that isn’t weight. A six-week trial in lean adults found breakfast eaters did not compensate by eating less later — but their physical activity thermogenesis was substantially higher, which largely cancelled the surplus, and their afternoon and evening blood sugar was more stable. “It doesn’t make you thin” is not the same as “it does nothing.”

What about the origin story?

Here we have to be careful, because the debunk is nearly as under-evidenced as the claim.

The standard account credits cereal marketing, usually via John Harvey Kellogg’s magazine or Edward Bernays’ 1925 bacon campaign. We can confirm that Bernays held the Beech-Nut Packing Company as a client in 1925 — it is in his papers at the Library of Congress. But the campaign as popularly told rests on Bernays’ own memoir, written some forty years after the fact by the man it flatters. No contemporaneous survey, press release or trade report corroborating it has surfaced.

And a full-text search of digitised books turns the phrase up in an American food magazine in 1914 — three years before the usual attribution and eleven before Bernays. There is no peer-reviewed historical tracing of the phrase at all.

So: the marketing story may well be right. It is not documented. We are flagging that because a magazine correcting its record should not swap one confident unsourced claim for another.

What this evidence cannot tell you

The randomised trials are short — seven trials, mean follow-up seven weeks, all rated at high or unclear risk of bias in at least one domain. A 0.44 kg difference over two months says little about a lifetime habit. Heterogeneity in the energy-intake result was severe.

The cohort studies cannot escape reverse causation: people who are unwell, working shifts, depressed or drinking heavily skip breakfast because of those things, and no amount of statistical adjustment fixes it. Notably, the heart-disease association was found to run largely through BMI, blood pressure, cholesterol and diabetes — which is what you would expect if breakfast skipping is a marker for a cluster of behaviours rather than a cause.

The summary

Breakfast is not the most important meal of the day. It is a meal. If you enjoy it, eat it; if you are not hungry, the evidence gives you no reason to force it. If you are trying to lose weight, adding a meal is a strange way to go about it.

The claim that was true for hungry children got sold to well-fed adults, and the scientific literature spent thirty years citing itself into believing it.
